Prepare questionnaires and inspect participation results separately from question design. Give each survey a purpose and a responsible reviewer.
A working example
Readiness example: ask who owns item data, which warehouse opening counts are approved and what is blocking the first receipt. Test the questions with an example participant, then review the responses with the implementation lead. A missing answer is an open decision, not a pass. Collect only the personal information necessary for the stated purpose.
